Abstract
PT Berlina Tbk is one of the manufacturing companies that processes plastic pellets into plastic bottle packaging products. Based on data from researchers obtained from October 2023 to November 2023, the 19-liter Gallon production process at PT Berlina.Tbk has several defects. During October 2023 to November 2023, these products had defects of 6,273 defective products or reached 3.19% of the total production of 196,348 (100%) 19-liter gallon products in October-November 2023.The 19-liter Gallon defect has several defects, namely Dirty Black, Dirty Die Head, Tonsil Mouth and Folding Bottom. This study uses a quantitative descriptive method by conducting field research activities, interviews, observations and studying several references such as books, journals and literature related to this research. Based on the analysis that has been carried out using FMEA (Failure Mode and Effect Analysis), the largest RPN values were produced in order from black dirty defects , dirty die heads, tonsil mouths, and folding bottoms in 19-liter Gallon products at PT Berlina.Tbk, which is then analyzed using FTA (Fault Tree Analysis) and produces several error factors, namely man, material, and machine factors. From the results of the analysis, it was announced that the Proposal to repair the Black Dirty Die Head defect, namely dismantling the Die Head to clean the scale on the Die Head, proposal to repair the Black Dirty defect, namely Be careful when filling and always close the reservoir after filling, Proposals for repairing defects in the Tonsil Mouth, namely handling the alignment of the parison gap and cleaning the die head so that there is no slip material, and proposals for defect repair Bottom Folding is to do preventive measures periodically on the teramat such as filling up water and always checking the teramat temperature.
